Well, it probably differs from what version of the game you are playing on. I'm currently playing on the Xbox version of this game, and I've heard that it's only possible for your companion to die if you kill them. But I'm not exactly sure, maybe she died while teleporting to me and I just wasn't paying enough attention.
It's a bit strange though, because Lydia seems to be completely blind to my efforts to evade pressure plates and tripwires.
More than once she has been sliced apart by dwarven blades, crushed by falling boulders, impaled by falling mammoth skulls, or burnt to a crisp in a gas explosion... only for her to bend over temporarily and eventually get back up and set off the next trap.

Giants are the only things I've seen that have killed her permanently. Unless she just got knocked so far away she stopped following me and I just never found her again. I've never seen her killed by Falmer, or any other beasties.

Also if anyone hasn't done Meridia's daedric quest, I'd advise you to because you get the most epic view of Skyrim ever when you talk to her.

I too have it on Xbox and started playing the game with lydia and now I'm level 55 or something like that with Lydia as my left hand with full duel enchanted daedric armor.

I've watched her die many, many, many times until I got the spell Courage and Rally and shit. If she gets knocked down to her knee heal her quickly if there is a mage near by. Ice Spike from arch mages will Kill her. After I took cover behind the wall during Azura's star, she was getting Ice spiked still trying to charge, but obviously walking super slow because her stamina was drained I tried to heal her without exposing myself because his Ice Wall staff did kind of a lot of damage, then she just dropped because I couldn't pull out my hands to heal her in time.

She also got destroyed by a Master Dwemer Centurion, One hit Steam Blast, I was surprised that just happened because I was selfish and was only healing myself and wasn't using the heal other spell.

These were times I was screwing around with Lydia and seeing what's her limit with different weapons. She died a few times during testing as did I. This was also when I discovered she can't equip daedric bows and then after that she only equips her hunting bow now. Right now she has a Ebony Warhammer (Epic) with Fire damage and Absorb health enchantments.

Also Traps can't kill her, I found that out when I first obtained her, And she does get killed by lingering effects from spells.

Do not use J'zargo's Flamecloak near her she dies almost instantly.

Meridias quest on that part reminded me of Knights of the Nine quest over White Gold tower

Well, either way, I never really enjoyed having Lydia as a follower. She always got in the way, and is probably the reason I don't have a current follower now.

Although I'm level 75, so it really doesn't matter if I have a follower anymore.
Just started making an Alchemist Conjurer a couple of days ago, Some Dunmir chick. Level 18 right now, no one as my follower and my potions are strong, Conjuring hasn't gone anywhere yet.
Yeah, there was one of these peaceful giants, and we were pushing him off this cliff into the river, when a hostile giant and mammoth attack. And the giant sends Lydia flying over a mountain, so I just waited (1 hour) and she was back.

Easy fix.
