I do agree with the whole "don't worry about the age and name idea", and cutting to the chase can also be a good thing, but I also think a bit of flair is necessary to add a bit of personality to the whole application process.
I would much rather read an interesting application from somebody trying to grab our attention and show us a bit of his personality, than read a cookie cutter app filled with what is most likely bullshit reasons for wanting in the clan.
I think what we need is to have them write a freeform application, with a few questions set up to have them tell us about themselves as a person. Than at the end have them tell us why they want into our clan.
