I'd have to say "real Mexican restaurant" because imo authentic food tastes WAY better than the cheap alternative at a fast food chain.
Cleanest fast food place has to be In-N-Out Burger. Clean ass bathrooms too.

My sister worked at a few places (Taco Bell, MCD, Magic Wok) and she said taco bell was actually really clean. I mean the worst part she said was that the meat was just a frozen bag of meat that the reheat. I for one don't like fast food but I love Subway (if that counts) and the Wendy's chicken sandwiches. But only the sandwiches.
I am not a big fan of eating at fast food restaurants or eating out at all. The only time I even eat at fast food restaurants is when i'm traveling somewhere.
Anyways, the cleanest fast food restaurant I've ever been to was Jack in a Box. The floor wasn't too dirty and the tables all looked usable. I am sure there are cleaner fast food restaurants, I just haven't been to many :P
