ok well I want to know how ishi got that little small ishi on his shoulder and is it possible if someone else could get it?
yes. there's one on the market for 1mil I think

The one on his shoulder he got as his custom item for being a Toribash Legend.
The ishi on the market linked above is a separate item that does nothing at all. A collectors item pretty much. ishi had a few made and gives them out for people who do him a favor or something, not too sure.
